How to fill Passport Application Forms

FILLING UP THE PASSPORT APPLICATION FORMS

Paste a photo in the Passport Application forms first page. (Recent color Photo showing full face, size 3.5cm x 3.5cm)

Paste two photos in the Passport Particulars Forms with cross signature half on form other half on Photo.

Fill the Payment Details for fees.

Signature (Blue / Black pen only)
Thumb impression for men its left hand and women its right hand (Blue / Black ink only).

All the instructions given below are to be read and followed very carefully. All these instruction are also applicable to Tatkal Passport Scheme as well.

COLUMN 1 – NAME,Example: BELLARY SHIVLOKA KRISHNA IYER, First Surname IYER comma then Name BELLARY SHIVLOKA KRISHNA in the remaining Columns.

If their is no surname name then Just write the name in Name Column leaving the Surname Column blank.

COLUMN 2 If thier is change in name, then fill it with previous name and new name like change in name or surname after marriage.

COLUMN 3 Indicate M for Male or F for Female or X for Others in the box.

COLUMN 4 – DATE OF BIRTH
Date of Birth should be in dd/mm/yyyy format as per the Local Muncipal or Government Authorities. Attach the Date of Birth Proof based on the secified list.

COLUMN 5 – PLACE & COUNTRY OF BIRTH
An Indian candidate whould mention the Village / Town, District, State of Birth Place. If the Candidate is born outside India just need to mention the Place of Birth, And if the candidate is born before Partition of India, which is now in Bangladesh or Pakistan, need to fill the Place Name as Undivided India

COLUMN 6, 7, & 8 – Name of Father, Mother & Spouse along with surnames are to be filled.  If the individual is not married then its NA – Not Applicable.

COLUMN 9 & 10 – Fill the details of Date of Residing at the Address given,Telephone Number along with Area Code and Mobile Number would be more useful for SMS update from Passport Officials. If the period of residence is less then one year while filling the Passport Application Forms then COLUMN 9 should be filled, the other Address proof of other addresses whould to be attached along the Passport Application Forms. Students can apply Passport using the Address Proof of the Study Place. They can also use the Bonafide Certificate from Institute. Passport Particulars Forms is to be filled as their are more then one Address.

COLUMN 11 – If their is any previous Passport or Passports then Passport Number / File Number has to be filled with Date of Issue and Place of Issue. In case the previous Passport was not received or issued then the details should be filled. All facts should be mentioned to aviod the penal provisions uner Section 12 of the Indian Passport Act, 1967.

COLUMN 11(a) – Individual who has travelled in emergency under Emergency Certificate (EC) or ever deported or repatriated to India at Government cost should give the details of EC Number, Place, Date of Issue with Original seizure memorandum, place and country from where deported or repatriated should be given in this Column. A Affidavit should be attached along with all details as such.

COLUMN 12 – Deatils of Educational qualifications, the visible distinguishing mark and height measured in centimeters should be filled.

COLUMN 13 – Central or State Government, Public Sector Undertakings, Statutory Bodies employees should fill details like Yes or No. Identity Certificate from concerned office should be attached as mentioned in Annexure B.

COLUMN 14 – Birth, Descent, Registration or Naturalization B/D/R/N with respect to citizenship should be mentioned in this Column.

COLUMN 15 – This Column is related to ECR Clearnace from Protector General of Emigrants for all Indian Citizen in case they want deemed to have been granted ECNR status an ECR Stamp. In case the passport booklet does not have ECR stamp, the indivdual would be deemed to have been granted ECNR status. No ECNR stamp will be affixed on the passport.An indivdual would be eligible for ECNR status if he/she falls in any one of the following categories and provides Documentary Proof thereof:

PERSONS ON WHOSE PASSPORTS ECR STAMPS WOULD NOT BE AFFIXED:

(I)

(a) All Diplomatic/official Passports holders.

(b) All the GAZETTED Government servants.

(c) All Income-Tax payers (including Agricultural Income-Tax Payees) in their individual capacity. Proof of assessment of income tax for last one year and
actual payment of income tax for last one year or copy of PAN card if required, and not merely payment of advance tax. Income tax returns stamped by
income tax authorities.

(d) All professional Degree holders, Doctors holding MBBS degree or equivalent degree in AYURVED or HOMEOPATHY, accredited Journalists,
Engineers, Chartered Accountants, Cost Accountants, Lecturers, Teachers, Scientists, Advocates, etc.

(e) Spouses and dependent children of category of persons listed from (b) to (d).

(f) All indivdual having educational qualifications of matriculation and above.

(g) Seamen who are in possession of CDC or Sea Cadets, Deck Cadets.

(i) Individual who have passed final examination of three years B.Sc. Nautical Sciences Courses at T.S. CHANAKYA, MUMBAI.

(ii)Individual who have undergone 3 months Pre-Sea training at any of the Government approved Training Institutes such as T.S.CHANAKYA, T.S. REHMAN, T.S. JAWAHAR, MTI (SCI) and NIPM, CHENNAI, after production of identity cards issued by the Shipping Master, MUMBAI/KOLKATA/CHENNAI.

(h) Persons holding Permanent Immigration Visa, such as the visas of UK, USA and Australia.

(i) Persons possessing two years diploma from any institute recognized by the National Council for Vocational Training (NCVT) or State Council of Vocational Training (SCVT) or persons holding three years’ diploma/equivalent degree from institutions like Polytechnics recognized by Central/State Governments.

(j) Nurses possessing qualifications recognized under the Indian Nursing Council Act. 1947.

(k) All individual above the age of 50 years.

(l) All persons who have been staying abroad for more than three years (the period of three years could be either in one stretch or broken) and their spouses.

(m) All children up to the age of 18 years of age. (At the time of re-issue at the age of 18 years, ECR stamping shall be done, if applicable)

(II) Their is No emigration clearance is required for visiting Pakistan, Bangladesh, and European countries in Europe (excluding CIS states), North America, Australia, Japan, New Zealand, South Korea, South Africa and Singapore.

COLUMN 16 – While applying for the first time for the minor children (less then 18 years). The details Parent’s Valid passports held by BOTH OR EITHER parents should be filled in this Column.

Annexure C For Children whose parents are not married

Annexure G For Consent of Parents Permission

Annexure H is to be flled and attached

Annexure I For Minors whose Parents do not hold Valid Passports

COLUMN 17 – Individual should fill all correct information. Suppression of any fact pertaining to all information may lead to fine up to Rs.5000 per offence and other penal provisions as Indivdual under the provisions of the Indian Passports Act, 1967 as amended from time to time.

COLUMN 18 – The Name and Address with Mobile or Telephone number and e-mail of Individual to be intimated in event of death or accident is to be furnished.

COLUMN 19 – A Self Declaration made by the Individual about owing allegiance to the sovereignty, unity and integrity of India, not voluntarily acquiring the citizenship or travel document from any other country etc should be filled in this Column. This declaration also states in filling true information in the form and an individual is aware that it is an offence under the Indian Passport Act 1967 for giving any wrong information or suppression of any material information in getting the passport. The applicant also declares that he has no other passport or travel document. Under the space provided, the Signature or Thumb Impression (left hand thumb impression for male and right for female) should be furnished along with date and place of application.

COLUMN 20 – A checklist of the Photocopies of all documents that are to be attached as enclosures along with the application form should be listed in the blank spaces provided and each document self attested by the individual.
